Responsibilities

\u2022 Conducts comprehensive evaluations, assesses the social, emotional and support needs of patients and their families, and sets measurable objectives and goals that are formulated in conjunction with the rehabilitation team. Adjusts treatment plan to achieve maximum results in independence and\/or function.
\u2022 Identifies and assists the patient and\/or family to utilize appropriate community resources to achieve measurable objectives. Utilizes special skills and judgment to resolve adverse social and emotional responses connected with the injury\/illness.
\u2022 Provides direct social work intervention and instructs and counsels the patients and family members in treating and coping with social and emotional responses connected with the injury\/illness.
\u2022 Regularly re-assesses effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment.
\u2022 Communicates patient\u2019s needs and progress to the treatment team, physician, patient, family members, and others as applicable.
\u2022 Consults with medical providers regarding patient evaluation & treatment, progress of the patient &discharge from treatment.
\u2022 Observes\/documents and reports changes in the condition\/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members.
\u2022 Maintains and submits evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations.
\u2022 Adheres to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state\/federal laws.
\u2022 Performs other duties as assigned.<\/p>

Qualifications

\u2022 Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC)
\u2022 Current Social Work or Marriage and Family Therapist licensure\/certification in the state(s) of practice required by individual states.
\u2022 A minimum of one year of experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury or other acute diagnosis.
\u2022 Current BLS (Basic Life Support) First Aid and CPR certifications.
\u2022 Demonstrates effective and professional interpersonal, verbal, and written communication skills.
\u2022 Able to work independently and part of a team.
\u2022 Demonstrates strong organization skills and attention to detail.<\/p>
